BMI alone does not determine surgical eligibility; it is evaluated together with the following clinical criteria.

Surgery may be considered when at least one obesity-related comorbidity such as type 2 diabetes, hypertension, sleep apnea, dyslipidemia or fatty liver disease is present.
Per the 2022 ASMBS/IFSO update, metabolic surgery may be considered in selected patients with type 2 diabetes uncontrolled despite medical therapy.
Patients aged 18-65 are typically preferred, though age alone is not an exclusion. Operative eligibility is determined after anesthesiology and surgical risk assessment.
